Registration for Children's Ministry

 
This year we are asking that all children be pre-registered. For you convenience, you can use this form to register all your children at one time for:
 
 Nursery Check-In, Sunday School (2yrs old - 6th grade),  Wednesday Night and First Friday.
 
The first section is for parental information (you only have to provide this information once).  Please fill out information on each of your children being sure to give their full name and what they prefer to be called. Please do not leave any field blank.  Please enter N/A if your child has no special needs.
